Origins Medicine & Aesthetics (OMA) Office Guidelines

FINANCIAL POLICY

Payment is due in full at the time of service unless otherwise arranged in advance. Treatment packages and memberships are non-refundable and non-transferable once purchased. If OMA determines that it is medically appropriate or necessary to discontinue the patient-provider relationship, any refund for unused prepaid services will be made at OMA's sole discretion and in accordance with applicable California law. As every patient and treatment is unique, individual results cannot be guaranteed and may vary.

CANCELLATION & APPOINTMENT POLICY

At OMA, we value your time and the dedicated time our clinical team reserves for each patient. To provide the highest level of care and respect the schedules of all patients, we kindly ask that any appointment changes or cancellations be made at least 24 business hours in advance.

Appointments cancelled or rescheduled with less than 24 business hours' notice may be subject to a cancellation fee. Patients who frequently reschedule appointments may be required to prepay for future appointments in order to reserve clinical time.

We recommend arriving 15 minutes prior to your scheduled appointment to allow time for check-in, paperwork, and preparation. If you arrive late, your appointment time may need to be shortened to avoid impacting other patients. In some cases, your appointment may need to be rescheduled, and you may be responsible for a partial or full service fee depending on the circumstances.

Missed appointments without prior notice (no-shows) will be subject to a booking fee of $150 or 10% of the scheduled service fee, whichever is greater.
